Job Description:

A company is looking for a Senior Care Manager (RN).

Key Responsibilities

Develops and assesses ongoing care/service plans, collaborating with providers to address member needs
Coordinates care between members, families, and providers to ensure appropriate person-centered services
Monitors and updates care plans based on member status and collaborates with stakeholders to meet goals

Required Qualifications

Active NY State RN license
Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
4 - 6 years of related experience
Knowledge of state and federal regulations related to healthcare
Experience in care management and developing care plans
