  • Job Requirements
  • Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals.
  • Documented proof of two MMR vaccines or Positive IGG Titer.
  • Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler.
  • Provider verification of proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster.
  • Valid 10 years from the administered date.
  • 1st PPD within 1 year, 2nd PPD within 30 days of start. And/or Negative IGRA within 90 days of start date lab printout prior to start.
  • Required for all travelers.
